Below are applications to complete to apply for a Provisional Membership to Las Amigas and Membership to Los Amigos.  Please submit forms online or print and mail the Provisional Membership Application  for each Las Amigas and Los Amigos to:

Las Amigas - For Parents

Annual Tuberculosis Test
Las Amigas who wish to volunteer at OIC must submit the results of an annual TB skin test.  If the skin test is positive, then a chest x-ray is required.  ​

Annual Dues
​Las Amigas annual dues are $65.00.  Please obtain an Invoice from the Treasurer before submitting payment. 

Los Amigos - Son(s)/Daughter(s)

Los Amigos Members who are fourteen (14) years old or older must submit the results of an annual TB skin test.  If the skin is positive, then a chest x-ray is required,

Annual Dues
Los Amigos annual dues are $75.00 per Member.  This includes supplies for two No-Sew Fleece Blankets to be donated to OIC and Las Amigas member polo shirt.  ​Please obtain an Invoice from the Treasurer before submitting payment.
